    Incidence And Factors Associated with Adverse Perinatal Outcomes in the Second Twin of Women Delivering at Kampala International University Teaching Hospitals

    Background. It was not known the exact Incidence of adverse perinatal outcomes in second twins of women who delivered at Kampala International University – Teaching hospitals, in Uganda. These hospitals had reports of twin deliveries every week with some adverse perinatal outcomes such as stillbirth, low APGAR score at 5 minutes, and neonatal deaths of second twins. This study was crucial to address the gaps and elucidate the incidence of adverse perinatal outcomes of second twin deliveries, to identify their types and the factors associated with adverse perinatal outcomes. Methods. Hospital - based prospective cohort study was conducted with 117 women twin pregnant who were admitted and delivered from Kampala International University-Teaching Hospitals during three months from November 2022 to January 2023. Consecutive sampling technique allowed to get data and data were entered in Excel 2019 then transferred to STATA 14.2 for analysis was according to objectives: frequencies, proportion and logistic regression run both in bivariate and multivariate. All variables with p value < 0.20 in bivariate logistic regression analysis fulfilled the criteria for multivariable logistic regression analysis; adjusted odds ratio with 95% confidence interval was used to measure the association variable with p < 0.05. Results. The incidence of adverse perinatal outcomes in second twins was 38.5%, fresh stillbirth accounted 12.8%, low APGAR score 24.0% and neonatal deaths 8.6 %. Fresh still birth was highly observed in peasant women (aOR: 12.69; 95%CI: 1.28-142.76, p = 0.040); vaginal delivery (aOR: 0.01; 95%CI: 0.00-0.50, p = 0.022) had protective properties for adverse fetal outcomes in second twin deliveries. Conclusions and Recommendations. Incidence of adverse fetal outcomes in second twins is high, and the occupation of women with twin pregnancy is highly a risk of adverse perinatal outcomes in second twins.
